Required Documents
Please gather these required documents to enroll your student.
Parent/Guardian Photo ID and proof of guardianship if applicable.
Birth Certificate – If you need assistance with obtaining a copy of your child's birth certificate please view the Vital Records Assistance information at the bottom of our Community page.
Proof of Residency – A recently dated gas, electric, trash, water, cable/internet, or utility bill; property tax bill; lease; mortgage statement; or renters or homeowners insurance. A driver’s license, cell phone bill, medical bills, DHHS paperwork, and other mail cannot be used as proof.
Immunization Records – A MCIR Consent Form must also be submitted with immunization records for new students moving to Portage from out-of-state. A Kalamazoo County Health Department Immunization Waiver or Medical Waiver may also be submitted in place of immunization records.
Hearing/Vision Screenings – Kindergarten/Young 5s only
Oral Health Screening – Kindergarten/Young 5s only
Current Transcript – High School students only
Online Enrollment
To complete our online enrollment, current families should log into Skyward Family Access to complete our New Student Online Enrollment. Families new to Portage will need to create an account first to be able to enroll online. Completing this application does not guarantee enrollment. The District will contact you once your application has been received to confirm enrollment. Please only complete this step if you live in Portage or have received formal approval to attend from outside of the district.
Before Your Student's First Day
Families are required to authorize enrollment in person before your student can start school. Additional steps for scheduling may be required for middle and high school students as well. Follow these guidelines for when to visit the school:
Elementary: Families are encouraged to visit the school between 9:30-11:30 a.m., 1:00-3:00 p.m., or by appointment.
Middle School: Families are encouraged to make an appointment with the school’s guidance office for course selection and student assessments, as needed.
High School: Families are required to make an appointment with the school’s guidance office to finalize enrollment.
