Due to FERPA laws, we cannot release school records to anyone but the former student (if over the age of 18) without their consent.
Current Students and Recent Graduates (most current year):
If you need a transcript for college, work, or scholarship applications, please use the Parchment Transcript Service, or contact your high school’s counseling office. To apply to in-state colleges, students must request their transcript through Parchment. Contact the counseling office of your high school for out-of-state college transcript requests, if you are a current student or a recent graduate. Note: Portage Public Schools does not keep diplomas, as they are a ceremonial document. Your transcript is your official document proving graduation. If you need additional records, like test scores, please indicate on your form. Thank you.
Students who graduated or attended prior to the most recent grad year, please DO NOT request from Parchment or your school. To request your transcript from the District office, please use the following form:
Portage Public Schools can provide an official transcript for Portage Public Schools graduates and attendees. The Record Release form will be your written authorization for Portage Public Schools to release your records. Your signature is required. Please allow up to five (5) business days for processing. Note: Portage Public Schools does not keep diplomas, as they are a ceremonial document. Your transcript is your official document proving graduation. If a photo of the above form is used, please make sure it is clear and legible.
