Portage Public Schools is no longer accepting paper copies of sports physicals. Families must upload their physical form AND register for the sport(s) you are interested in via Final Forms.

Physicals must be dated on or after April 15th, 2023 to count for the 2023-2024 school year and submitted through Final Forms by the first day of tryouts/practice in order to participate.
